You are responsible for all fees associated with services provided. Payment is due at the time of service unless otherwise agreed upon. Insurance estimates are not guarantees of payment. Any balance not covered by insurance is the patient’s responsibility.

Message frequency varies. Message and data rates may apply. Reply HELP for support. Reply STOP to opt out.
2. Message Frequency - You may receive up to 4 messages per month, depending on your interaction with our office.
